Registering as a Patient


register_now_blue_signWe are pleased to welcome new patients who live within our practice area.  Please bring your medical card or, alternatively, download our registration form below.

To prevent fraud in the NHS, patients wishing to register with the practice are now required to provide two forms of identification before registering; as examples, either a passport, driving license, birth certificate, etc, plus a form of identification to prove your current address, for example, a utility bill.

New patients are encouraged to attend for a health check with one of our Healthcare Assistants. If you have repeat prescriptions you will need to be seen by a doctor to discuss your medications.

PreRegistrationIf you wish to register with the Practice, click on the link below to open the registration form. Once you have printed and completed the form please bring it to the surgery.  You will be asked to provide two forms of identification when registering as a new patient.  The first form of identification must confirm who you are, i.e. driving license, passport, etc.  The second form of identification should confirm your registering address, e.g. utility bill.

When you register you will also be asked to fill out a New Patient Questionnaire. This is because it can take a considerable time for us to receive your medical records. The form can be accessed via the link below.  Please complete this form and bring with you to your New Patient Medical appointment with our Nurse.
